Police are urgently searching for Shoaa Asmar, 27, who took her baby son Omar from an address on Croft Avenue in Merrylands about 6.30pm on Monday.
It is believed Ms Asmar left with her son in a dark blue 1998 model Toyota Corolla with black and gold-coloured NSW plates UZS-116.
She is described as being 165 centimetres tall, with tanned skin, medium build and brown eyes.
She was last seen wearing a dark coloured jilbab (a scarf covering her torso and head). She also regularly wears a hijab.
A police spokeswoman said they held "grave concerns for their safety".
Members of the public should ring triple-0 with any information on the whereabouts of mother and son.
